Titration

A highly experienced consultant psychiatrist will conduct a thorough evaluation of your ADHD. This includes a complete medical history and an assessment using the standard ADHD questionnaires and rating scales.

Following your assessment you will be informed whether medication may be beneficial in your particular situation. This will be discussed at your follow up appointment (which could be in person, via video link or by phone). Your doctor will explain the different options available for you to take medication and, if you choose to begin taking medication, we will suggest titrating the dose to maximize the benefits/side effects balance for you.

This process can take several weeks, and it involves gradually altering your child's medication until the appropriate balance is reached. It's not uncommon to have to titrate up to three different medicines before you can find the one that best suits your child's ADHD. Parents must take part in this process of titration and keep a close eye on their child's behaviour so that they can let the doctor know if the medicine is helping or causing side effects.

If you decide to go back to NHS treatment, you will receive a private prescription for your medication. You will be required to pay the pharmacy. This is not included in the consultation fee. You must keep up with your appointments every six months to monitor your medication. This includes the measurement of pulse and blood pressure. Annual reviews are also required.

Follow-up appointments

Once the assessment has been completed, you will be able to make appointment with the psychiatrist, as appropriate. The appointment is usually done by video conference using zoom. In some cases, it may be possible to have an audio-only consultation to be conducted (again with an additional cost).

At this point, we will discuss any treatment options and agree upon a bespoke individualised ADHD medication titration strategy. This will include a review and discussion of your medication dosage, as well as any side effects that you may be experiencing. The titration sessions cost PS150 for face-toface sessions and PS60 by video or phone consultation. The pharmacy will charge you for any medication costs.

The doctor will also ask you about your family history and the impact that your ADHD symptoms impact your life, including any issues with your relationships that you may be experiencing or other mental health problems that you may be experiencing. ADHD can be a genetic condition, and it's vital to assess your family history in order to determine if you are at a higher chance of developing this disorder.

During this examination, it is vital that you are truthful and open with the doctor, as they will use the information you provide to determine whether you are able to meet the diagnostic criteria for ADHD. This is a crucial step, since an incorrect diagnosis could lead to inappropriate medication prescription.

If you have decided that you'd like to receive treatment for your ADHD We will arrange a follow up appointment with one of our psychiatrists who specialise in this field. This can be done within the first few days of your diagnosis if you want to start treatment right away or within two weeks if you prefer to take time to consider the implications of your diagnosis and the options available to you.

We suggest that if you are unsure of your preferences, you speak with your GP prior to booking your private assessment. Some providers have different guidelines regarding whether or not they require a GP letter. We also recommend that you check whether your GP accepts a "Shared-Care Agreement', which means that once you are discharged from the clinic, you will be sent back to them to get the prescription for medication.

Medication

For many people who want an individual diagnosis of ADHD, medication is a key element of their treatment. Finding prescriptions from an NHS psychiatrist can be a challenge and long-winded. https://www.iampsychiatry.uk/private-adult-adhd-titration/ waiting lists are notoriously long, and GPs may be reluctant to refer patients on to specialist services.

Requesting your GP to refer you to Right to Choose is one way to avoid this (see psychiatry - UK and ADHD 360). This means that you can choose a provider that offers an assessment and titration. This can cut down on wait time by a significant amount and is more convenient if you're working and have a hectic schedule.

Once you've been referred, your clinician will examine your condition to determine if you need medication titration. You will need to complete forms that are different for each provider however the most commonly used ones are self-reporting forms and medical histories that go to childhood and adulthood. These will help your clinician identify any factors that might mean you shouldn't take ADHD medications if they are prescribed.

If you are titrated on medication, it will typically include a mix of non-stimulant and stimulant medications. The combination will be carefully tested and monitored to determine the most effective combination for you, taking into account any existing conditions you have. The titration process can be lengthy and may require a number of additional appointments before the dose becomes stable.

The frequency of your follow-up appointments will be contingent on the type and amount of medication that you are taking. They can be scheduled for monthly or quarterly. The appointment will involve your doctor checking to see if you are taking the correct medication, logging any side effects you have had and making any necessary adjustments to ensure that you remain in a safe dosage. You will also be required to attend a routine health check, which could include blood pressure and pulse readings, as well as weight and weight measurements and an annual review.

For some individuals it could be because they are satisfied with their medication and don't require a further discussion about their titration. If you're looking to discuss your titration or treatment plan in more detail it is likely that you'll be sent back to the private clinic who made your initial assessment. If this happens, the titration appointment will be part of their consultation fees and they will receive their medication via private prescription (see Consultation fees above). This can cost more than an NHS prescription because the pharmacy will charge for the medication.
